Capacity Assessments

Through the Capacity Assessment Office of the Public Guardian and Trustee, and the Substitute Decisions Act of the Province of Ontario:

I am a designated Capacity Assessor on the list with the Ontario Ministry of the Attorney General. In this role, I perform assessments on individuals who may lack cognitive capacity to continue managing their personal finances and/or personal care.

Individuals contract with me to be assessed themselves, or to have a family member assessed.

Some of the issues that may require an assessment include:

• Alzheimer's or dementia
• Cognitive capacity reduced due to a motor vehicle or some other accident
• Stroke
• Neurological impairment

Assessments are usually conducted in the person's home, or in a long term care facility. Sometimes I am asked to see the client in a general hospital or group home.

I am available to travel within a 100km radius of Toronto to perform these assessments.

I also consult with hospitals and other institutions on the level of independence that is appropriate for a patient with cognitive impairment.
